How We Work Together
Step 1: Schedule Your Appointment
There are several ways that you can schedule your first appointment. You can book online here, email the office, or phone for an appointment 236 766 1763
Step 2: Register For Access To Your Online Patient Portal
Once you’ve made your appointment, you’ll be given the opportunity to create a patient profile through my online patient portal, which will provide you with access to intake forms, patient documents, treatment plans, and handouts. Please register an account using the same email address that you used to schedule an appointment. Once you’ve registered, you’ll be given access to complete you new patient intake forms.
Step 3: Complete Your New Patient Intake Forms
Once you’ve created your online portal patient profile, you will be asked to complete your new practice member intake forms. Please give yourself at least one to two hours to finish this. You don’t have to complete it in one sitting, so feel free to come back to it to add more, change your answers, etc. Be honest about where you’re at right now and we’ll get you to where you want to be. It gives me valuable information and insight into your history and health journey before we even meet.
Step 4: Your First Appointment
This is the first and most important step of our work together. At our first appointment, we’ll take a detailed medical history and review any previous lab work you might have and work together determining what exactly is the most important to you. We’ll also review your current diet, medications, and supplements. Then we make decisions on any lab testing that will be critical to your treatment by uncovering information about exactly how your body is functioning.
Step 5: Your Lab Tests
Lab tests can be any combination of blood, urine, saliva, or stool samples. Some are done locally at Valley Medical, some are done in office, and some are done at home. You’ll leave knowing exactly which tests we’ll be running, and why they’re so important to your health goals. Labs take anywhere from 1 – 4 weeks to get the results back, depending on the test.
Step 6: Your First Follow-Up Appointment
The first follow-up appointment is typically scheduled for four to eight weeks after your first visit. Most times, we will be able to review some of the lab work that has been done, and develop an initial treatment plan. At subsequent visits, we’ll review any other labs and integrate those into your plan. Your first treatment plan will typically cover the next 8 – 12 weeks.
Step 6: Your On-Going Health Care
Subsequent visits involve measuring, and tracking the progress you’ve made in restoring balance in your body. We will re-run individual tests to make sure that we are on the right track and if needed, update and revise treatments. At the completion of your active treatment program, we will design an on-going wellness program to be reviewed and updated every few months. These appointments are usually scheduled monthly as brief check-in appointments to monitor your progress.
